A pillow changes the position of the head and neck for hours at a time. That makes height and shape relevant, but it does not make every “orthopaedic” claim true.

What the evidence suggests

A 2021 systematic review and meta-analysis found that spring and rubber pillows may improve neck pain, waking symptoms, disability and pillow satisfaction in adults with chronic neck pain. It also noted that cervical alignment can be affected by pillow shape and height. The studies varied, which means the result does not prove that one contour or material works for everyone.

Fit matters more than labels

  • Side sleepers often need enough height to fill the space between shoulder and head.
  • Back sleepers often prefer a lower contour that supports the neck without pushing the head forward.
  • Mattress firmness changes how far the shoulder sinks and therefore changes the pillow height required.
  • Adjustability can be valuable because body size and comfort differ.

A sensible trial

Begin with the lower contour, check that your face points forward rather than tilting, and assess comfort over several nights. A new sensation is possible; escalating pain, numbness, weakness, severe headache or breathing difficulty is a reason to stop and seek advice.
